This church was formerly the seat of the city senate established by Charles V, with Polizzi having the right to sit in the Sicilian Parliament (among the oldest in the world) since it was the king's city.

Built in the 14th century, it was initially dedicated to S. Maria del Soccorso. It was later called S. Maria lo Piano because it stands on Piano Trinità, the quintessential square in Polizzi.
The church is one of the 'FAI's places of the heart' and contains roof trusses made from the wood of the Abies Nebrodensis, only about 30 specimens of which are still alive around the world, with secondary designs depicting small monsters and animals.
Also noteworthy is the main altar embellished with pure gold bas-reliefs on a cobalt background.
