The church stands opposite the Chiesa Madre and was built on the remains of an earlier hospital. This is the home of the Confraternity of San Gandolfo, the patron saint of the city.

Built in 1622 to house the Confraternity of San Gandolfo (established or renewed in 1501). Attached to the church is the convent, which was inhabited first by the Collegine di Maria and then by the Franciscan Sisters of the Bambin Gesù until 1992.
The single-nave church houses several valuable works, including a canvas depicting the Immaculate Conception and one showing scenes from the life of San Gandolfo painted by De Matta.
The painting of 1620 by Giuseppe Salerno depicting the Patronage of San Gandolfo. It contains one of the few depictions of Polizzi Generosa from that century, with the castle, city gates and wall and churches no longer standing.
